Document Requirement For Maid Application

1. 2 pieces passport size photos of husband and wife.

2. Photocopy of IC husband and wife.

3. Photocopy of marriage certificate.

4. Photocopy of birth certificate for children under the age of 15 years old.

5. ** Copy B Form with payment receipt to LHDN (if self employed)
OR BE Form / EA Form with Employment Letter and Latest 3 Months Payslip.

6. Photocopy of electricity bill with current address.



Application for 2nd Maid or Taking Care of Elderly

1. Copy of IC mother or father.

2. Copy of birth certificate husband or wife (who related with the elderly).

3. Original medical letter from hospital or clinic.
